Job Description

This per diem nurse practitioner role is structured for flexibility and autonomyperfect for those who value independent practice and adaptable scheduling.

Schedule:

  • Design your own schedule with shifts available Monday through Sunday
  • Choose flexible hourly commitments, set weekly or bi-weekly
  • Assignments will be scheduled within Kernersville, Clemmons, Lewisville, and nearby areas

Key Responsibilities:

  • Conduct detailed annual home health risk assessments in patients' homes, each lasting approximately 45 minutes
  • Evaluate prior and current medical history, assess ongoing medical conditions, and review medication regimens
  • Deliver clear patient education regarding medical conditions and medication usage
  • Identify care gaps and provide actionable recommendations for improved patient outcomes
  • Focus solely on assessmentsno treatment planning or prescribing required

Qualifications:

  • Active Nurse Practitioner license in North Carolina
  • Board Certification via AANP or ANCC
  • Current CPR (BLS) certification
  • Minimum of 6 months of current nurse practitioner experience preferred
  • MSN degree from an accredited nursing program
