Seize the opportunity to make a meaningful impact by performing comprehensive home health risk assessments for Medicare Advantage plan members throughout Forsyth County, NC. This per diem nurse practitioner role is structured for flexibility and autonomyperfect for those who value independent practice and adaptable scheduling.
Schedule:
- Design your own schedule with shifts available Monday through Sunday
- Choose flexible hourly commitments, set weekly or bi-weekly
- Assignments will be scheduled within Kernersville, Clemmons, Lewisville, and nearby areas
Key Responsibilities:
- Conduct detailed annual home health risk assessments in patients' homes, each lasting approximately 45 minutes
- Evaluate prior and current medical history, assess ongoing medical conditions, and review medication regimens
- Deliver clear patient education regarding medical conditions and medication usage
- Identify care gaps and provide actionable recommendations for improved patient outcomes
- Focus solely on assessmentsno treatment planning or prescribing required
Qualifications:
- Active Nurse Practitioner license in North Carolina
- Board Certification via AANP or ANCC
- Current CPR (BLS) certification
- Minimum of 6 months of current nurse practitioner experience preferred
- MSN degree from an accredited nursing program
