Job Description

Enjoy the autonomy and flexibility of per diem work while impacting patient wellness as a Nurse Practitioner specializing in home health. This role centers on conducting comprehensive risk assessments for Medicare Advantage members in Randolph County, NCincluding Asheboro, Archdale, and Trinity. Visits typically last about 45 minutes and encompass both annual health assessments and post-discharge check-ins. Youll be scheduled in various locations across the county, providing vital information and health guidance to members in their homes.

Schedule:

  • Flexible shifts available seven days a week
  • Set your own availability
  • Minimum weekly or bi-weekly hourly commitments

Responsibilities:

  • Perform in-home annual risk assessments and comprehensive health evaluations for Medicare Advantage members
  • Review and assess patient medical history, chronic conditions, and medication management
  • Educate patients on their medical conditions and prescribed medications
  • Identify care gaps and make recommendations to promote better outcomes
  • No requirement to diagnose, prescribe, or create formal treatment plans

Ideal Qualifications:

  • Active Nurse Practitioner license in North Carolina
  • Certified by either AANP or ANCC
  • Current Basic Life Support (CPR) certification
  • Masters degree in Nursing (MSN) from an accredited institution
  • At least 6 months recent Nurse Practitioner experience is preferred
