Take on a meaningful healthcare role focused on delivering comprehensive home health risk assessments to Medicare Advantage plan members. This per diem opportunity allows Nurse Practitioners to provide vital care and education through in-home visits across various communities within Custer County, including Weatherford, Arapahoe, Clinton, and Thomas. Each risk assessment typically takes around 45 minutes and may involve annual health evaluations or post-hospitalization visits.
Schedule:
- Flexible shifts available between Monday and Sunday
- Self-schedulingdesign your own work calendar
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Perform annual home health risk assessments for Medicare Advantage members
- Review and evaluate patients current and prior medical history and medications
- Educate patients on health conditions and proper medication usage
- Identify care gaps and recommend solutions to improve patient outcomes
- No treatment planning or medication prescribing required
Qualifications:
- Active Nurse Practitioner license in Oklahoma
- Current CPR (BLS) certification
- Board certification through AANP or ANCC
- At least 6 months of recent NP experience (preferred)
- Masters degree in Nursing (MSN) from an accredited institution
