Job Description

Step into a per diem opportunity as an experienced Nurse Practitioner conducting home health risk assessments for Medicare Advantage plan members across Sumter County. In this position, you will have the autonomy to customize your own schedule, with flexible shifts available seven days a week. Locations include communities within Sumter County such as The Villages, Wildwood, and Bushnell, providing a dynamic and rewarding environment for patient care.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Create your own schedule to fit your lifestyle
  • Minimum weekly or bi-weekly hour commitment

Responsibilities:

  • Perform comprehensive annual health risk assessments in members homes
  • Review prior and present medical history, ongoing conditions, and medications
  • Deliver patient education about medical conditions and prescribed medications
  • Identify care gaps and recommend interventions for best health outcomes
  • No requirement for creating treatment plans or prescribing medications

Qualifications:

  • Active Nurse Practitioner License in Florida
  • Current CPR (BLS) certification
  • Board Certification through AANP or ANCC
  • 6 months or more of recent NP experience preferred
  • Masters degree in Nursing (MSN) from an accredited institution
