Take on a meaningful per diem role where your expertise as a Nurse Practitioner will have a direct, positive impact on the health and well-being of Medicare Advantage plan members. In this position, you will conduct comprehensive home health risk assessments throughout Sumter County, providing valuable insight into the health status and needs of individuals in the community.
Schedule:
- Flexible shifts available Monday through Sunday
- Self-schedulingdesign shifts that fit your lifestyle
- Minimum weekly or bi-weekly hourly commitment required
Key Responsibilities:
- Perform annual home health risk assessments for Medicare Advantage plan members
- Thoroughly review medical history, current conditions, and medication regimens
- Deliver education to members about their conditions and proper medication use
- Identify gaps in care, escalating recommendations as appropriate
- Visits typically last around 45 minutes and may include annual health assessments or post-discharge follow-ups
- Responsible for assessments onlyno treatment planning or prescription tasks required
Qualifications:
- Valid Nurse Practitioner license in Florida
- Current BLS/CPR certification
- Board certified through AANP or ANCC
- Minimum of 6 months current Nurse Practitioner experience preferred
- MSN degree from an accredited nursing program
As part of a supportive team, youll be compensated at $80-$90 per completed evaluation, with a mileage stipend and administrative time also provided.
