Job Description

Deliver essential care to patients in their homes as an experienced Nurse Practitioner. This per diem opportunity is centered on performing detailed home health risk assessments for Medicare Advantage plan members within Marion County, FL. Your clinical visitseach averaging about 45 minutesmay involve annual health assessments or post-discharge evaluations. Service areas include Ocala, Belleview, Dunnellon, and surrounding communities, with shift assignments varying within these locations as needed.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Design your own work schedule
  • Commitment required on a weekly or bi-weekly basis

Primary Responsibilities:

  • Conduct comprehensive risk assessments in clients homes for Medicare Advantage members
  • Review and evaluate current and prior medical history, health conditions, and medication usage
  • Educate members regarding their medical conditions and medication regimens
  • Detect gaps in care and recommend interventions without the responsibility to diagnose, create treatment plans, or prescribe medications

Required Qualifications:

  • Active and unrestricted Nurse Practitioner license in Florida
  • Board Certification through AANP or ANCC
  • Master of Science in Nursing (MSN) from an accredited institution
  • Current Basic Life Support (BLS/CPR) certification
  • 6 months or more of recent Nurse Practitioner experience preferred

This role is compensated at $80$90 per completed evaluation, along with a mileage stipend and administrative time.
