Seeking experienced Nurse Practitioners to perform home health risk assessments as part of a per diem opportunity. This position focuses on Medicare Advantage plan members residing throughout Clay County, including the areas of Brazil, Clay City, and Knightsville. Youll play a critical role in supporting patient wellness and closing care gaps while maintaining a flexible schedule that fits your lifestyle.

Schedule:

• Flexible shifts available Monday through Sunday
• Create your own work schedule
• Minimum hourly commitment required weekly or bi-weekly

Key Responsibilities:

• Conduct comprehensive home health risk assessments, typically lasting 45 minutes
• Evaluate patients medical and medication histories, both past and current
• Educate patients on their health conditions and prescribed treatments
• Identify gaps in care and provide actionable recommendations to address them
• Administer post-discharge and annual health assessments
• No responsibilities for creating treatment plans or writing prescriptions

Qualifications:

• Valid Nurse Practitioner license in Indiana
• Current CPR certification (BLS)
• Board Certification via AANP or ANCC
• MSN degree from an accredited nursing school
• Minimum of 6 months recent experience as a Nurse Practitioner preferred
