Job Description

Deliver high-impact care to community members through rewarding, flexible per diem work as a nurse practitioner specializing in home health assessments. In this role, youll perform home-based risk evaluations for Medicare Advantage plan participants throughout Vigo County, including the areas of Terre Haute, West Terre Haute, and Seelyville. Visit types vary and may include annual health assessments or post-discharge check-ins. Each assessment typically takes 45 minutes, allowing you to leverage your expertise while maintaining autonomy over your schedule.

Schedule & Flexibility

  • Choose your own shifts: set your availability between Monday and Sunday
  • Enjoy minimum weekly or bi-weekly hourly commitment
  • Assignments may vary by day within Vigo County locations

Core Responsibilities

  • Conduct annual home health risk assessments for health plan members at their residence
  • Review and evaluate previous/current medical conditions and medication history
  • Offer education on diagnoses, health management, and medications
  • Identify and report potential gaps in care, making thoughtful recommendations without creating or managing treatment plans
  • No prescribing or treatment plan development necessary

Qualifications & Experience

  • Active Nurse Practitioner license in Indiana
  • Board Certification (AANP or ANCC)
  • Current CPR (BLS) certification
  • MSN from an accredited nursing program
  • Minimum of 6 months current nurse practitioner experience preferred
