This per diem opportunity offers nurse practitioners the flexibility to conduct home health risk assessments for Medicare Advantage plan members across Clay County, IN, including Brazil, Clay City, and Knightsville. Each assessment typically takes about 45 minutes and may involve annual wellness checks or post-discharge visits. The focus is on evaluating patient history, providing education, and identifying care needsno prescribing or treatment plan development required.
Schedule:
- Flexible shifts available Monday through Sunday
- Self-scheduling to fit your availability
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Perform home health risk assessments in patient residences
- Review prior and existing medical history, chronic conditions, and medications
- Educate patients about their health conditions and prescribed medications
- Identify care gaps and recommend next steps to improve patient outcomes
- Complete thorough documentation without the need for prescribing medication
Qualifications:
- Active Nurse Practitioner license in Indiana (IN)
- Board certified by AANP or ANCC
- Current CPR (BLS) certification
- Graduate of an accredited MSN program
- At least 6 months of experience as a Nurse Practitioner is preferred
