As a Nurse Practitioner specializing in home health, you will conduct comprehensive risk assessments for Medicare Advantage plan members across McMinn County, TN. This role allows you to create your own schedule, with shifts available throughout the weekincluding weekends. Each assessment visit, lasting approximately 45 minutes, may involve annual health evaluations or post-discharge check-ins.
Schedule:

Responsibilities:
- Perform annual home health risk assessments for Medicare Advantage members
- Evaluate patient medical history, current conditions, and medication records
- Educate patients on managing their medical conditions and prescription use
- Identify gaps in care and offer actionable clinical recommendations
- Focus on assessment and educationno responsibility for developing treatment plans or prescribing medications
Qualifications:
- Active Nurse Practitioner license in Tennessee
- Board Certification (AANP or ANCC)
- MSN from an accredited school of nursing
- Current CPR (BLS) certification required
- Minimum 6 months experience as a practicing NP (preferred)
