Experienced Nurse Practitioners are sought for per diem home health risk assessment visits within McMinn County, TN. This role involves engaging with Medicare Advantage plan members across communities such as Athens, Etowah, and Englewood, providing a unique opportunity to deliver high-impact care directly in patients residences.
Schedule:
- Flexible shifts available Monday through Sunday
- Self-scheduling to suit your availability
- Minimum weekly or bi-weekly hourly commitment required
Key Responsibilities:
- Conduct annual home health risk assessments for Medicare Advantage members
- Review and evaluate patients prior/current medical history, conditions, and medication use
- Educate members regarding ongoing health conditions and prescribed medications
- Identify care gaps and offer recommendations to promote wellness and continuity of care
- No creation of treatment plans or prescription management required
Qualifications:
- Active Nurse Practitioner license in the state of Tennessee
- Board Certification through AANP or ANCC
- Valid CPR certification (BLS)
- Graduate of an accredited MSN program
- At least 6 months of recent Nurse Practitioner experience preferred
