Play a vital role in patient-centered care by conducting home health risk assessments for Medicare Advantage plan members throughout Seminole County. As a Nurse Practitioner in this per diem opportunity, youll make a direct impact on members health and wellness from the comfort of their homes. Assignments may take you to various communities across Seminole County, such as Wewoka, Bowlegs, and Konawa, offering both professional diversity and meaningful engagement.

Key Responsibilities:

  • Perform annual home health risk assessments for Medicare Advantage plan participants
  • Review each patients past and present medical history, current medical conditions, and medication usage
  • Educate members about their health conditions and medication regimens
  • Identify care gaps and provide recommendations to improve overall health outcomes
  • No requirement to prescribe medications or develop treatment plans

Qualifications:

  • Active Nurse Practitioner license in Oklahoma
  • Current BLS certification (CPR)
  • Board certified by AANP or ANCC
  • Six months of recent NP experience (preferred)
  • Masters degree in Nursing (MSN) from an accredited institution
