Step into a dynamic role as a Nurse Practitioner specializing in home health risk assessments for Medicare Advantage plan members throughout Columbia County, AR. Schedule:
- Flexible shifts available Monday through Sunday
- Customize your own work schedule
- Choose a minimum commitment on a weekly or bi-weekly basis
Key Responsibilities:
- Complete thorough annual home health risk assessments during 45-minute in-home visits
- Review patients' medical history, current conditions, and medication logs
- Educate patients regarding diagnoses, ongoing health maintenance, and prescribed therapies
- Identify care gaps and offer actionable recommendations to improve patient well-being
- Participate in both annual check-ups and post-hospital discharge visits
- No requirement to develop treatment plans or prescribe medications
Qualifications:
- Active Nurse Practitioner license in Arkansas
- Current CPR (BLS) certification
- Board certification via AANP or ANCC
- Completion of an MSN program from an accredited school of nursing
- At least 6 months of recent NP experience preferred
