Job Description
Leverage your clinical expertise as a Nurse Practitioner specializing in home health by providing risk assessments for Medicare Advantage plan members across Nash County. This per diem opportunity offers true flexibilityset your own schedule with shifts available any day of the week. Youll conduct home health risk assessmentstypically lasting about 45 minutesat patient residences throughout Rocky Mount, Nashville, and Red Oak. Each visit may vary in focus, including annual health checks or post-discharge assessments, helping to ensure patients receive comprehensive, personalized care in their own homes.
Schedule:
- Flexible shifts available Monday through Sunday
- Self-scheduling with the ability to create your own hours
- Minimum weekly or bi-weekly hourly commitment
Responsibilities:
- Conduct annual risk assessments for Medicare Advantage plan members in their homes
- Review and evaluate current and previous medical histories, health conditions, and medication usage
- Offer education to patients regarding medical conditions and medications
- Identify care gaps and recommend appropriate next steps (without writing treatment plans or prescriptions)
Qualifications:
- Nurse Practitioner license, valid in North Carolina
- Board Certification as a Nurse Practitioner (AANP or ANCC)
- Current CPR (BLS) certification
- MSN degree from an accredited nursing school
- At least 6 months of recent NP experience preferred
