Job Description
Deliver high-quality patient care and support preventative health initiatives as an experienced Nurse Practitioner specializing in home health risk assessments. This per diem position offers the opportunity to create your own schedule with flexible shifts available any day of the week, allowing for a dynamic work/life balance. You will complete comprehensive home health assessments for Medicare Advantage plan members located within Edgecombe County, including Tarboro, Lower Town Creek, and Princeville. Each in-home visit typically takes about 45 minutes and may involve annual health evaluations or post-discharge assessments.
Schedule:
- Flexible shifts between Monday and Sunday
- Self-schedulingchoose when you work
- Minimum weekly or bi-weekly hourly commitments available
Key Responsibilities:
- Conduct thorough annual or episodic in-home health risk assessments
- Review medical histories, current conditions, and medication use
- Educate patients on managing their conditions and medication regimens
- Identify care gaps and recommend solutions, enhancing patient outcomes
- No need to prescribe medications or develop care plans
Qualifications:
- Active Nurse Practitioner license in North Carolina
- Board certification via AANP or ANCC
- Current BLS (CPR) certification
- MSN from an accredited nursing program
- At least 6 months of recent Nurse Practitioner experience preferred
