Job Description

Step into a vital healthcare role where your expertise as a Nurse Practitioner can directly impact patient well-being through in-home health risk assessments. This per diem opportunity allows you to design your own schedule, supporting Medicare Advantage plan members throughout Leon County, Florida, including the Tallahassee area. Each assessment averages around 45 minutes, focusing on annual health evaluations or post-discharge visits in various residential settings across the county.

Schedule:

  • Flexible shifts offered Monday through Sunday
  • Self-scheduling for maximum convenience
  • Minimum weekly or bi-weekly hour commitments available

Key Responsibilities:

  • Complete comprehensive home health risk assessments for Medicare Advantage health plan participants
  • Review and evaluate prior and current medical history, existing conditions, and medication regimens
  • Educate individuals and their families on medical conditions, medication usage, and health maintenance
  • Detect potential gaps in care and make recommendations to ensure comprehensive health coverage and follow-up
  • Note: This role does not involve diagnosis, the establishment of treatment plans, or medication prescribing

Desired Qualifications:

  • Valid Florida Nurse Practitioner License
  • Board Certification through AANP or ANCC
  • Master of Science in Nursing from an accredited institution
  • Current CPR (BLS) certification
  • At least 6 months of recent nurse practitioner experience preferred
