A unique opportunity is available for Nurse Practitioners skilled in home health to deliver care and conduct risk assessments throughout Washington Parish, Louisiana. This per diem position is ideal for those seeking flexible scheduling, patient diversity, and independent practice within communities such as Bogalusa, Franklinton, and Angie.
Schedule:
- Flexible shifts available Monday through Sunday
- Self-scheduled shifts to match your availability
- Commitment options for weekly or bi-weekly hours
Key Responsibilities:
- Conduct comprehensive annual home health risk assessments for Medicare Advantage plan members in Washington Parish
- Evaluate patients prior and current medical history, ongoing conditions, and medication use
- Provide clear education on health conditions and medications as needed
- Identify and report care gaps, recommending actionable solutions
- Note: No requirement for creating ongoing treatment plans or writing prescriptions
Qualifications:
- Valid and active Nurse Practitioner license (State of Louisiana)
- Current BLS/CPR certification
- Board Certification by AANP or ANCC
- Minimum of 6 months current Nurse Practitioner experience preferred
- Master of Science in Nursing (MSN) from an accredited institution
Compensation for this role includes $80$90 per completed evaluation, along with a stipend for mileage and compensation for associated administrative time.
