Embark on a rewarding per diem opportunity delivering quality care as a Nurse Practitioner specializing in home health risk assessments. This flexible position allows you to make a direct impact in the lives of Medicare Advantage plan members across Thurston County, including Lacey, Olympia, and Tumwater.
Schedule:
- Flexible shifts available Monday through Sunday
- Design your own work schedule
- Minimal weekly or bi-weekly hourly commitment required
Key Responsibilities:
- Perform comprehensive annual home health risk assessments for Medicare Advantage members
- Review and evaluate medical history, chronic conditions, and medication lists during each visit
- Provide clear education to patients on their health conditions and prescribed medications
- Identify care gaps and suggest practical, actionable recommendations to help close them
- All visits are assessment-only with no requirements for developing treatment plans or prescribing medications
Qualifications:
- Active and unrestricted Nurse Practitioner license in Washington State
- Current Basic Life Support (BLS/CPR) certification
- Board Certification through the AANP or ANCC
- Preferred: At least 6 months of recent nurse practitioner experience
- Master of Science in Nursing (MSN) from an accredited institution
