Seeking an experienced Nurse Practitioner to provide compassionate home health risk assessments for Medicare Advantage plan members. This per diem position offers the flexibility to create your own work schedule with shifts available any day of the weekMonday through Sunday. Locations will vary within Columbus County, including Tabor City, Chadborn, Lake Waccamaw, and nearby areas. Assessments take approximately 45 minutes each, and duties may include annual health evaluations or post-discharge visits.
Schedule:
- Flexible shifts available Monday through Sunday
- Ability to create your own schedule
- Minimum required hourly commitment weekly or bi-weekly
Key Responsibilities:
- Complete comprehensive annual home health risk assessments for Medicare Advantage members
- Review and evaluate medical history, current conditions, and medication lists
- Educate patients regarding their conditions and prescribed medications
- Identify gaps in care and make appropriate recommendations
- No responsibility for treatment plan development or medication prescribing
Qualifications:
- Active Nurse Practitioner License in North Carolina
- Current BLS CPR certification
- Board Certification through AANP or ANCC
- MSN from an accredited school of nursing
- At least 6 months of current Nurse Practitioner experience preferred
