Job Description

This per diem opportunity offers exceptional flexibility, allowing you to design your schedule and choose shifts that best suit your availability, with assignments available in areas such as Salina, Onondaga, and Clay.

Schedule:

  • Flexible shifts available between Monday and Sunday
  • Create your own schedule based on your availability
  • Minimum weekly or bi-weekly hourly commitment required

Key Responsibilities:

  • Conduct in-home annual health risk assessments for Medicare Advantage plan members
  • Assess prior and current medical history, existing medical conditions, and medication usage
  • Educate patients on disease management, preventive care, and medication use
  • Identify potential gaps in care and provide actionable recommendations for improvement
  • No treatment planning or medication prescribing required

Qualifications:

  • Active Nurse Practitioner license in the state of New York
  • Current CPR certification (BLS)
  • Board certification through AANP or ANCC required
  • Minimum of 6 months of current NP experience preferred
  • Master of Science in Nursing (MSN) from an accredited institution
