A rewarding per diem opportunity is available for an experienced Nurse Practitioner specializing in home health assessments. In this role, you will perform comprehensive home health risk evaluations for Medicare Advantage members throughout Otero County, including communities such as La Junta, Rocky Ford, and Fowler. This flexible position allows you to design your schedule with options to work any day of the week.
Schedule:
- Flexible shifts from Monday through Sunday
- Choose your own hours
- Minimum weekly or bi-weekly hourly commitments as needed
Primary Duties:
- Conduct annual home health risk assessments in patients homes
- Review prior and current medical history, conditions, and medications
- Offer guidance on medical conditions and medication regimens
- Locate gaps in care and suggest solutions to promote wellness
- Complete documentation for each evaluation (approximately 45 minutes per visit)
- Note: No treatment plan creation or prescription responsibilities required
Qualifications:
- Unrestricted Nurse Practitioner license in Colorado
- Active board certification through AANP or ANCC
- Current BLS (CPR) certification
- MSN from an accredited nursing program
- At least six months of recent Nurse Practitioner experience preferred
