Step into an important role supporting community health by performing home health risk assessments for Medicare Advantage plan members. This per diem opportunity is ideal for a nurse practitioner seeking autonomy and flexibility, as youll visit patients in Otero County, including La Junta, Rocky Ford, and Fowler, ensuring quality care and education in their home environments.

Key Responsibilities:
- Perform comprehensive home health risk assessments for Medicare Advantage patients
- Evaluate and document prior and current medical histories, conditions, and medication use
- Educate patients on existing medical conditions and appropriate medication usage
- Identify care gaps and recommend targeted solutions
- No need to create treatment plans or prescribe medications
Qualifications:
- Active Nurse Practitioner license in Colorado
- Current CPR certification (BLS)
- Board Certification from AANP or ANCC
- Minimum of 6 months NP experience preferred
- MSN from an accredited nursing program
