As a Nurse Practitioner specializing in home health, you will perform focused health risk assessments for Medicare Advantage plan members in various McMinn County communities. Each assessment typically takes about 45 minutes and may include comprehensive annual evaluations or post-discharge follow-ups, providing an essential service to patients in their own homes.
Schedule:
- Flexibility to choose your own shifts, Monday through Sunday
- Set your minimum weekly or bi-weekly hourly commitment for optimal balance
Key Responsibilities:
- Conduct in-home annual health risk assessments for Medicare Advantage members
- Review and document prior/current medical histories, ongoing conditions, and prescribed medications
- Deliver patient education regarding diagnoses, conditions, and medication management
- Identify care gaps and make relevant recommendations to improve overall patient health
- No duties involving treatment plan creation or medication prescribing
Qualifications:
- Unrestricted Nurse Practitioner license in Tennessee
- Active CPR certification (BLS required)
- Board certification via AANP or ANCC
- Minimum six months of Nurse Practitioner experience preferred
- Graduate of an accredited Master of Science in Nursing (MSN) program
