Job Description
Deliver essential healthcare directly to patients by conducting in-home health risk assessments for Medicare Advantage plan members in Onslow County, NC. This per diem opportunity allows you to utilize your Nurse Practitioner expertise while enjoying the freedom to create your own schedule and select shifts that work best for you.
Schedule:
- Flexible shifts available Monday through Sunday
- Create your own schedule
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Conduct comprehensive home health risk assessments, lasting approximately 45 minutes per visit, for Medicare Advantage plan members
- Perform annual health assessments and post-discharge visits as needed
- Evaluate and document medical history, current conditions, and medication regimens
- Educate patients on their health conditions and medications
- Identify care and medication gaps, offering recommendations for follow-up
- No creation of treatment plans or prescribing required
Qualifications:
- Active Nurse Practitioner license in the state of North Carolina
- Current CPR (BLS) certification
- Board Certification through AANP or ANCC
- Minimum of 6 months experience as a Nurse Practitioner preferred
- MSN degree from an accredited school of nursing
Enjoy compensation of $80$90 per evaluation, along with a mileage stipend and pay for administrative time.
