Key Responsibilities:

  • Conduct comprehensive annual home health risk assessments for Medicare Advantage health plan participants
  • Evaluate prior and current medical history, ongoing medical conditions, and medication usage
  • Educate patients on their medical conditions and medication regimens, supporting health literacy and informed care
  • Identify care gaps and share targeted recommendations to improve patient well-being
  • No requirement to develop treatment plans or prescribe medications

Qualifications:

  • Active Nurse Practitioner License in North Carolina
  • Board Certification via AANP or ANCC
  • Current CPR (BLS) certification
  • MSN from an accredited nursing program
  • At least 6 months of current Nurse Practitioner experience preferred

In this role, you will be assigned to assess patients throughout various communities within Onslow County, including Jacksonville, Holly Ridge, and Swansboro. Each patient's home visit takes approximately 45 minutes, encompassing both annual assessments and post-discharge visits as needed.

This position offers compensation of $80-$90 per evaluation, along with a mileage stipend and compensation for administrative time, making your commitment both professionally and financially rewarding.
