Job Description

An exciting per diem opportunity is available for a skilled Nurse Practitioner with home health experience to make a meaningful impact through individualized care. This position centers on conducting home health risk assessments for Medicare Advantage members across Dearborn County, including communities such as Lawrenceburg, Greendale, and Aurora. By helping members understand their health status and supporting them in managing their conditions, you play a pivotal role in promoting wellness throughout the community.

Schedule and Flexibility

  • Flexible shifts available seven days a week
  • Create your own personalized schedule
  • Choose from minimum weekly or bi-weekly hourly commitments

Key Responsibilities

  • Perform annual home health risk assessments for Medicare Advantage members
  • Review and evaluate medical history, current conditions, and medication use
  • Educate individuals on their health conditions and prescribed treatments
  • Identify and document care gaps with recommendations for follow-up
  • There is no requirement to write treatment plans or prescribe medications

Qualifications

  • Active Nurse Practitioner license in Indiana
  • Board Certification through AANP or ANCC
  • Current CPR (BLS) certification
  • MSN from an accredited nursing program
  • At least 6 months of recent NP experience in a relevant setting preferred
