Deliver vital healthcare services in the community as a Nurse Practitioner specializing in home health risk assessments. This per diem opportunity offers professionals autonomy and variety, conducting visits with Medicare Advantage plan members in Dearborn County. Assignments take place throughout Lawrenceburg, Greendale, Aurora, and surrounding towns, supporting patients post-discharge or through annual assessments in the comfort of their homes.
Schedule:
- Flexible shifts available seven days a week
- Self-scheduling to fit your availability
- Minimum commitment required, either weekly or bi-weekly
Key Responsibilities:
- Perform annual home health risk assessments for Medicare Advantage health plan members
- Assess and evaluate medical history, current health conditions, and medication use
- Educate patients about their diagnoses and prescription regimens
- Identify care gaps and provide recommendations to address them
- No requirements for prescribing medications or developing treatment plans
Qualifications:
- Valid Nurse Practitioner license in Indiana
- Current BLS CPR certification
- Board certification via AANP or ANCC
- MSN degree from an accredited nursing program
- At least six months of recent NP experience preferred
