This per diem opportunity offers the flexibility to design your own schedule, with shifts available any day of the week. You will conduct thorough home health risk assessments, supporting patients in Concord, Hooksett, Bow, and their neighboring communities.

Schedule:

• Flexible shifts between Monday and Sunday
• Create your own schedule
• Choose minimum weekly or bi-weekly hourly commitments

Key Responsibilities:

• Complete annual home health risk assessments for Medicare Advantage members in the comfort of their homes
• Evaluate prior and current medical history, ongoing conditions, and medication regimens
• Educate patients about their medical conditions and medications, ensuring understanding and compliance
• Identify potential gaps in care and recommend solutions
• Focus exclusively on assessmentno requirement for creating treatment plans or prescribing medications

Qualifications:

• Valid Nurse Practitioner license in New Hampshire
• Board Certification through AANP or ANCC is required
• Current CPR certification (BLS)
• MSN from an accredited nursing school
• At least 6 months of recent NP experience is preferred
