As a Nurse Practitioner specializing in home health, you will make a direct impact by conducting risk assessments for Medicare Advantage plan members throughout Washoe County, NVincluding areas like Reno, Sun Valley, and Spanish Springs. This per diem opportunity offers remarkable flexibility, allowing you to design a schedule that fits your lifestyle.

Schedule:

• Flexible shifts available Monday through Sunday
• Self-scheduling; choose your preferred days and hours
• Set your own minimum weekly or bi-weekly commitment

Key Responsibilities:

• Complete thorough home health risk assessments (typically 45 minutes each) during scheduled visits, which may include annual assessments or post-discharge follow-ups
• Review prior and current medical history, medical conditions, and medication records
• Offer clear education to patients regarding their health conditions and prescribed medications
• Identify gaps in care and make recommendations for further evaluation or follow-up as appropriate
• Focus exclusively on assessments; there is no requirement to create treatment plans or prescribe medications

Qualifications:

• Active Nurse Practitioner license in Nevada
• Current CPR certification (BLS)
• National Board Certification through AANP or ANCC
• Preferred minimum of 6 months of recent Nurse Practitioner experience
• MSN degree from an accredited nursing program
