Make a difference in community health by delivering in-home risk assessments and wellness evaluations. This per diem opportunity is ideal for Nurse Practitioners seeking flexibility while working across Dane County, including Madison, Sun Prairie, and Fitchburg. Choose your own schedule, with flexible shifts available seven days a week and the option to set your minimum weekly or bi-weekly hours.

Key Responsibilities:

• Conduct comprehensive home health risk assessments for Medicare Advantage members
• Review prior and current medical history, health conditions, and medication use
• Educate patients on health management and medication adherence
• Identify care gaps and recommend appropriate follow-up actions
• Perform annual wellness or post-discharge assessments (no ongoing treatment planning or prescription required)

Qualifications:

• Active Nurse Practitioner license in Wisconsin
• Current CPR certification (BLS)
• Board Certification through AANP or ANCC
• Minimum of 6 months recent NP experience preferred
• MSN degree from an accredited nursing program

Schedule:

• Flexible shifts between Monday and Sunday

• Ability to set a minimum weekly or bi-weekly hourly commitment
