Enjoy the flexibility and autonomy of providing high-quality home health risk assessments to Medicare Advantage members across Monroe County, FL. Practitioners in this per diem opportunity will travel to the homes of members in diverse communities, such as Key West, Key Largo, and Marathon, performing comprehensive evaluations that help inform overall health management and care plans.
Schedule:
- Flexible shifts available any day, Monday through Sunday
- Self-schedulingset your own hours
- Minimum weekly or bi-weekly hourly commitment to fit your lifestyle
Key Responsibilities:
- Perform thorough annual risk assessments in member homes, each typically lasting about 45 minutes
- Review prior and current medical histories and medication usage
- Provide patients with clear education regarding their conditions and prescribed medications
- Identify potential care gaps and recommend simple steps for resolution
- No requirements for treatment planning or medication prescriptionfocus on assessment and guidance only
Qualifications:
- Active Nurse Practitioner License in Florida
- Current BLS/CPR certification
- Board Certification through AANP or ANCC
- MSN degree from an accredited nursing school
- At least 6 months of nurse practitioner experience preferred
As a valued practitioner, youll receive $80-$90 per evaluation, along with a mileage stipend and compensation for administrative timeempowering you to maximize your skills while being fairly rewarded.
