Explore a highly rewarding opportunity as a Nurse Practitioner specializing in Home Health, providing impactful care throughout Monroe County. As an essential member of a dynamic home health team, you'll conduct risk assessments for Medicare Advantage plan members residing in communities including Key West, Key Largo, and Marathon. Each home visit is structured, focused, and typically lasts about 45 minutesranging from annual health assessments to post-discharge reviews.
Schedule:
- Flexible shifts available, Monday through Sunday
- Autonomy to create your own schedule
- Minimum commitment required weekly or bi-weekly
Primary Responsibilities:
- Perform comprehensive home health risk evaluations for Medicare Advantage members
- Review and assess patient medical histories, current conditions, and medication lists
- Deliver clear, patient-focused education regarding health conditions and medication regimens
- Identify gaps in patient care and offer actionable recommendations
- No development of treatment plans or prescribing responsibilities required
Qualifications:
- Current and active Florida Nurse Practitioner License
- Board Certification through AANP or ANCC
- Valid CPR certification (BLS)
- MSN degree from an accredited nursing program
- At least 6 months recent experience as a Nurse Practitioner preferred
