Seeking a proactive and compassionate clinician to provide critical home health risk assessments to Medicare Advantage members in the community. This per diem opportunity is ideally suited for Nurse Practitioners looking for schedule flexibility and autonomy while making a valued impact in patient care.
Schedule:
- Flexible shifts available Monday through Sunday
- Ability to create your own schedule
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Perform comprehensive annual health and post-discharge risk assessments in patients homes across several locations in the region
- Review patient histories, chronic conditions, and medications to gather a holistic clinical picture
- Educate patients on disease processes, medication adherence, and general health maintenance strategies
- Detect care gaps and recommend solutions to optimize patient well-being
- No requirements for establishing treatment plans or prescribing medications
Qualifications:
- Current, unrestricted Nurse Practitioner license in Florida
- Valid CPR (BLS) certification
- Board Certification through AANP or ANCC
- MSN degree from an accredited nursing school
- Minimum of 6 months of experience as a Nurse Practitioner preferred
This role offers a competitive compensation structure of $80-$90 per completed evaluation, with mileage stipends and additional administrative time included. You will have the opportunity to work with Medicare Advantage plan members in diverse communities throughout Sumter County, including The Villages, Wildwood, and Bushnell, with each assessment typically lasting about 45 minutes.
