Step into a meaningful per diem opportunity as a Nurse Practitioner focused on delivering home health risk assessments within Marion County, FL. This role empowers you to use your clinical expertise in a flexible setting, conducting comprehensive 45-minute evaluations for Medicare Advantage plan members in locations including Ocala, Belleview, and Dunnellon.
Schedule:
- Flexible shifts available Monday through Sunday
- Design your own schedule according to your availability
- Minimum commitment required weekly or bi-weekly for selected shifts
Key Responsibilities:
- Perform annual and post-discharge home health risk assessments at member residences
- Review and document prior and current medical history, ongoing conditions, and medication regimens
- Educate patients about their conditions and medication management to promote better outcomes
- Assess gaps in care and offer recommendations to support patient health
- Role is assessment-only: treatment planning and prescribing are not required
Qualifications:
- Active Nurse Practitioner license in the state of Florida
- Current Basic Life Support (BLS) certification
- Board Certification through AANP or ANCC
- Graduate of an accredited Master of Science in Nursing (MSN) program
- At least 6 months of recent Nurse Practitioner experience preferred
