Job Description

Experienced Nurse Practitioners are sought for per diem home health risk assessments. This dynamic opportunity allows providers to work within Greene County, performing comprehensive visits in a variety of community settings, including Linton, Bloomfield, Jasonville, Stockton Township, and nearby areas. Flexible scheduling and autonomy over shifts make this position ideal for motivated clinicians who thrive outside the traditional clinic environment.

Schedule:

  • Flexible shifts available, Monday through Sunday
  • Self-scheduling: create your own calendar
  • Minimum required commitment on a weekly or bi-weekly basis

Key Responsibilities:

  • Perform risk assessments for Medicare Advantage plan members in their homes
  • Review and evaluate prior/current medical history, ongoing conditions, and medication use
  • Educate patients on their health conditions and prescribed medications
  • Detect care gaps and provide recommendations to improve health outcomes
  • No requirement to create treatment plans or prescribe medications

Qualifications:

  • Valid Nurse Practitioner license in Indiana
  • Active CPR certification (BLS)
  • Board certified by AANP or ANCC
  • At least 6 months of nurse practitioner experience preferred
  • Master of Science in Nursing (MSN) degree from an accredited institution

This role compensates $80$90 per completed evaluation, and also offers a mileage stipend and coverage for administrative time. The work is structured for maximum flexibility and efficiencyaverage home visits last about 45 minutes.
