Schedule:

• Flexible shifts available Monday through Sunday
• Craft your own schedule with minimum weekly or bi-weekly hourly commitments

Key Responsibilities:

• Conduct home health risk assessments, typically lasting 45 minutes, in a variety of settings including annual health evaluations and post-discharge visits
• Review and assess prior and current medical history, conditions, and medication usage
• Educate Medicare Advantage members on their medical conditions and prescribed medications
• Identify and address gaps in care, providing clear recommendations for follow-up
• Note: This position does not require creating treatment plans or prescribing medications

Qualifications:

• Current Nurse Practitioner license in Virginia
• Active CPR certification (BLS)
• Board certified through AANP or ANCC
• At least 6 months of current experience as a Nurse Practitioner preferred
• Master of Science in Nursing (MSN) from an accredited program

This position delivers a competitive earning potential of $80-$90 per evaluation, with a mileage stipend and compensation for administrative time.
