Job Description

Enhance the quality of patient care by performing comprehensive home health risk assessments throughout Lincoln County, Oklahoma. This flexible, per diem Nurse Practitioner opportunity involves traveling to the homes of Medicare Advantage plan members in communities such as Chandler, Stroud, and Prague. Your role is pivotal in fostering patient well-beingeach assessment averages 45 minutes, focusing on annual health reviews or post-discharge evaluations.

Schedule:

  • Flexible shifts available any day of the week (MondaySunday)
  • Self-directed schedulingbuild your own calendar
  • Minimum hourly commitment on a weekly or bi-weekly basis

Key Responsibilities:

  • Conduct thorough home health risk assessments for Medicare Advantage members
  • Review and evaluate patients medical histories, ongoing conditions, and medications
  • Educate members about their health conditions and prescribed medications
  • Detect potential care gaps and recommend strategies to resolve them
  • No requirement to create treatment plans or prescribe medications

Qualifications:

  • Active Nurse Practitioner license in Oklahoma
  • Board certification through AANP or ANCC
  • Current CPR (BLS) certification
  • MSN from an accredited school of nursing
  • At least 6 months of Nurse Practitioner experience preferred
