Job Description

A unique opportunity is available for skilled Nurse Practitioners interested in delivering quality care through home health risk assessments. Work per diem with the flexibility to create your own schedule and choose shifts across the week, including weekends. Youll be serving Medicare Advantage plan members throughout Limestone County, including areas such as Mexia, Groesbeck, and Coolidge. Each risk assessment visit, averaging 45 minutes, focuses on comprehensive evaluations without the responsibility to prescribe or initiate treatment plans.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Design your own schedule
  • Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

  • Conduct thorough annual or post-discharge in-home risk assessments
  • Review and evaluate members prior/current medical history, ongoing conditions, and medication usage
  • Provide education to members regarding their medical conditions and medications
  • Identify gaps in care and make thoughtful recommendations to address them
  • Documentation and completion of each assessment as specified
  • No prescribing or treatment planning required

Qualifications:

  • Current Nurse Practitioner license in the state of Texas
  • Active CPR certification (BLS required)
  • Board Certification via AANP or ANCC
  • Master of Science in Nursing (MSN) from an accredited institution
  • At least six months of recent Nurse Practitioner experience preferred

Compensation includes $80-$90 per evaluation completed, with an additional mileage stipend and payment for administrative time.
