Make a meaningful impact in patient care by providing comprehensive home health risk assessments for Medicare Advantage plan members. This per diem Nurse Practitioner opportunity involves conducting detailed visits with patients residing in the Winchester, VA area. Each assessment lasts approximately 45 minutes and focuses on evaluating overall health, prior and current medical history, and ensuring any gaps in care are addressed.
Schedule:
- Flexible shifts available Monday through Sunday
- Choose your own schedule
- Minimum weekly or bi-weekly hourly commitment required
Key Responsibilities:
- Complete annual health assessments for Medicare Advantage members in their homes
- Review and evaluate medical history, existing conditions, and medications
- Educate patients about their health conditions and medication regimens
- Identify care gaps and provide actionable recommendations for improved patient outcomes
- Visits may include annual assessments or post-discharge check-ins
- No requirement to create treatment plans or prescribe medications
Qualifications:
- Current Nurse Practitioner license in Virginia
- Active Basic Life Support (BLS) certification
- Board Certification through either AANP or ANCC
- At least 6 months of recent Nurse Practitioner experience is preferred
- Master of Science in Nursing (MSN) from an accredited program
