Step into an impactful role providing essential home health risk assessments for Medicare Advantage members across Warren County, VA, including areas such as Front Royal and Bentonville. As a Nurse Practitioner specializing in home health, you will support patient well-being through comprehensive evaluations conducted in members' homes. This per diem position offers a high degree of autonomy and flexibility, making it ideal for those seeking to customize their work hours around other professional or personal responsibilities.
Schedule/Hours:
- Flexible shifts available Monday through Sunday
- Self-scheduling to suit your lifestyle
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Complete annual and post-discharge risk assessments in patient homes
- Review and document prior/current medical history, conditions, and medication regimens
- Offer education about medical conditions and medications to patients
- Detect gaps in care, suggesting solutions for optimal health outcomes
- Focus on assessment and education only, with no treatment planning or prescription responsibilities
Qualifications:
- Active Nurse Practitioner license in VA
- Board Certification via AANP or ANCC
- Current CPR certification (BLS)
- At least 6 months of recent NP experience preferred
- MSN degree from an accredited nursing school
Compensation includes $80$90 per evaluation, along with a mileage stipend and compensation for administrative time.
