Job Description
We are seeking an experienced Nurse Practitioner to perform in-home health risk assessments for Medicare Advantage plan members throughout Portage County. This per diem opportunity allows for exceptional flexibilityyou have the option to create your own schedule, with shifts available any day of the week. Assignments may occur in Kent and other nearby communities including Ravenna, Aurora, and Streetsboro.
Schedule:
- Flexible shifts offered Monday through Sunday
- Design your own work schedule
- Commit to a minimum number of hours each week or bi-weekly
Key Responsibilities:
- Conduct comprehensive home health risk assessments (approximately 45 minutes per visit)
- Review medical, medication, and treatment history to ensure thorough documentation
- Educate members on conditions, medications, and general health practices
- Identify and report any care gaps or concerns, offering practical recommendations for follow-up
- Focus exclusively on assessment; no treatment plan development or prescription responsibilities
Qualifications:
- Valid Nurse Practitioner (NP) license in the state of Ohio
- Current CPR certification (BLS)
- Board Certification through either AANP or ANCC
- Minimum of 6 months of NP experience preferred
- Master of Science in Nursing (MSN) from an accredited institution
Compensation: Nurse Practitioners are compensated $80-$90 per evaluation, in addition to a mileage stipend and reimbursement for administrative time.
