Make a positive impact in your community by conducting comprehensive home health risk assessments within Windham County, CT. As a Nurse Practitioner specializing in home health, youll visit Medicare Advantage plan members in their homesthese may include annual wellness exams or post-discharge follow-ups and typically take about 45 minutes per appointment. This flexible, per diem role offers an opportunity to use your clinical expertise to deliver essential care and education directly to patients in areas such as Windham (Willimantic), Killingly, and Mansfield.
Schedule:
- Flexible shifts available between Monday and Sunday
- Set your own schedule to fit your personal and professional commitments
- Minimum weekly or bi-weekly hourly requirement
Key Responsibilities:
- Conduct annual home health risk assessments for Medicare Advantage health plan members
- Review patients medical history, current health conditions, and medication regimen
- Educate patients on health issues, conditions, and the importance of medication adherence
- Identify gaps in care and offer actionable recommendations to address needs
- No requirement for treatment planning or prescription writing
Qualifications:
- Valid Nurse Practitioner license in Connecticut
- Board Certification through AANP or ANCC
- Current BLS (CPR) certification
- Master of Science in Nursing from an accredited institution
- A minimum of six months current NP experience preferred
