Job Description

Make a meaningful impact in the community by providing comprehensive home health risk assessments to Medicare Advantage plan members throughout Windham County, CT. This per diem role is ideal for Nurse Practitioners who thrive on autonomy, enjoy flexible scheduling, and are passionate about preventative care.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Design your own work schedule
  • Commit to minimum weekly or bi-weekly hours as fits your availability

Responsibilities:

  • Complete annual home health risk assessments for Medicare Advantage members
  • Review and assess medical history, current conditions, and medication lists
  • Educate individuals on their health conditions and prescribed medications
  • Identify care gaps, providing recommendations for improved health outcomes
  • Visits may vary, including annual assessments or post-discharge evaluations
  • No responsibilities for treatment planning or prescribing medications

Candidates may be assigned to diverse communities within Windham County (including Windham/Willimantic, Killingly, and Mansfield) for each shift, ensuring variety and breadth in the patient population served.

Qualifications:

  • Active Nurse Practitioner license in Connecticut
  • Current CPR (BLS) certification
  • Board certification through AANP or ANCC
  • Minimum of 6 months of current Nurse Practitioner experience preferred
  • Master of Science in Nursing (MSN) from an accredited institution
