An exceptional per diem opportunity is available for experienced Nurse Practitioners to perform home health risk assessments for Medicare Advantage plan members throughout Windham County. This flexible role is ideal for professionals who value autonomy and appreciate making a difference in a community setting. Each home health assessment averages approximately 45 minutes and may include either an annual health assessment or post-discharge visit. Assessments will be conducted in various Windham County locations, offering diversity in your daily routine and patient interactions.
Schedule:
- Flexible shifts available Monday through Sunday
- Create your own work schedule
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Perform comprehensive home health risk assessments for Medicare Advantage members
- Review and evaluate patients prior and current medical history, conditions, and medication use
- Provide thorough education about medical conditions and medications
- Identify and address gaps in care, providing recommendations for improvement
- No responsibility for initiating treatment plans or prescribing medications
Qualifications:
- Current Nurse Practitioner License in Connecticut
- Active CPR (BLS) certification
- Board Certification via AANP or ANCC
- Six months or more of recent NP experience preferred
- Master of Science in Nursing (MSN) from an accredited institution
