Job Description

Make a meaningful impact by providing personalized care as a Nurse Practitioner specializing in home health risk assessments. This per diem opportunity allows you to enjoy maximum flexibility while serving Medicare Advantage plan members across locations within Idaho County, such as Grangeville, Cottonwood, and Kamiah. Youll conduct 45-minute assessments that vary between annual health evaluations and post-discharge visits, with all scheduling focused around your availability and preferred geographic area.

Schedule:

  • Flexible shifts available Monday through Sunday

  • Minimum weekly or bi-weekly hourly commitment, tailored to your needs

Responsibilities:

  • Complete comprehensive annual home health risk assessments for Medicare Advantage members
  • Review and evaluate clients medical histories, current conditions, and medications
  • Educate patients on their health conditions and medication management
  • Identify gaps in care, providing recommendations for improvement
  • No requirement to prescribe medications or devise treatment plans

Qualifications:

  • Valid Nurse Practitioner license in the state of Idaho
  • Board Certification through AANP or ANCC
  • Master of Science in Nursing (MSN) from an accredited school
  • Current CPR certification (BLS)
  • Minimum of 6 months of recent NP experience preferred
