Advance your career by delivering essential care directly to patients homes in Whiteside County, Illinois. This per diem opportunity is ideal for a Nurse Practitioner with a passion for home health and a desire for flexibility. You will conduct home health risk assessments for Medicare Advantage plan members residing in Sterling, Rock Falls, Morrison, and surrounding areas within Whiteside County. Each visit takes approximately 45 minutes and can include annual health assessments or post-discharge care.

Schedule:

• Flexible shifts available Monday through Sunday
• Design your own work schedule
• Minimum weekly or bi-weekly hourly commitments available

Key Responsibilities:

• Perform thorough annual home health risk assessments for Medicare Advantage members
• Review and evaluate prior and current medical history, conditions, and medications
• Provide patient education on health conditions and medication management
• Proactively identify care gaps and recommend actionable solutions
• Visits will not require treatment plans or prescribing medications

Qualifications:

• Active Nurse Practitioner license in Illinois
• Board Certification from AANP or ANCC
• Current CPR (BLS) certification
• MSN degree from an accredited nursing program
• At least 6 months of recent NP experience is preferred

Compensation consists of $80$90 per evaluation, with a mileage stipend and additional pay for administrative time to ensure your expertise is well rewarded.
