Schedule:

• Flexible shifts available Monday through Sunday
• Control your own schedule
• Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

• Perform annual home health risk assessments for Medicare Advantage health plan members
• Assess and document prior and current medical history, active medical conditions, and medication use
• Deliver thorough education on medical conditions and treatment plans to patients
• Note and address any gaps in ongoing care, recommending appropriate resources or interventions
• Focus exclusively on assessmentno requirement to prescribe medications or develop treatment plans

Qualifications:

• Active Nurse Practitioner licensure in Virginia
• Board Certification through AANP or ANCC
• Current CPR (BLS) certification
• 6 months or more of recent Nurse Practitioner experience preferred
• MSN degree from an accredited institution

Compensation and Benefits:

• Earn $80-$90 per evaluation
• Receive a mileage stipend for travel
• Compensation for administrative time related to documentation
