Discover a rewarding per diem opportunity for an experienced Nurse Practitioner to provide high-impact home health risk assessments throughout Ouachita County. Schedule:

• Flexible shifts available Monday through Sunday

• Minimum weekly or bi-weekly hourly commitment required

Key Responsibilities:

• Complete annual and post-discharge risk assessments in patients homes, primarily within Ouachita County, which may include Camden, Stephens, and Bearden
• Review and document prior and current medical histories, conditions, and medication usage
• Deliver evidence-based education regarding health conditions and prescribed medications
• Detect gaps in care and make recommendations to encourage continued patient wellness
• No requirements for developing treatment plans or prescribing medications

Qualifications:

• Unencumbered Nurse Practitioner license in Arkansas
• Active CPR certification (BLS)
• Board certification through AANP or ANCC
• Minimum of 6 months recent experience as an NP preferred
• Master of Science in Nursing (MSN) from an accredited institution

This per evaluation opportunity offers $80-$90 per assessment, in addition to a mileage stipend and compensation for administrative time.
