Experienced Nurse Practitioners are sought to perform home health risk assessments for Medicare Advantage plan members throughout Staunton and the surrounding communities within Augusta County, including areas such as Craigsville and Grottoes. This per diem opportunity offers a highly flexible schedulechoose shifts Monday through Sunday, and set your own weekly or bi-weekly hourly commitment to fit your lifestyle.

Role Overview:
As a Nurse Practitioner specializing in Home Health, you will:

• Conduct comprehensive annual and post-discharge risk assessments in patients homes (each visit approx. 45 minutes)
• Review and evaluate medical histories, health conditions, and medication regimens
• Educate patients about their diagnoses and treatments
• Identify care gaps and communicate recommendations to improve health outcomes
• No prescribing medications or devising treatment plansfocus is strictly on assessment and education

Qualifications:

• Active Nurse Practitioner License in Virginia
• Current BLS/CPR certification
• Board Certification through AANP or ANCC
• At least six months of recent NP experience preferred
• Master of Science in Nursing (MSN) from an accredited institution

Compensation & Perks:
