This per diem opportunity is ideal for a Nurse Practitioner seeking flexibility and autonomy, with the chance to create your own schedule and serve residents across Staten Island and greater New York, NY.

Schedule:

• Flexible shifts available Monday through Sunday
• Create your own schedule to fit your lifestyle
• Minimum weekly or bi-weekly hourly commitment required

Key Responsibilities:

• Conduct thorough annual home health risk assessments for Medicare Advantage plan members during home visits
• Review and document patients previous and current medical histories, including chronic conditions and all medications
• Educate patients about their health status, ongoing conditions, and medication regimens
• Identify and address gaps in care, recommending follow-up steps as appropriate
• Administrative tasks may be necessary; no prescribing or creation of treatment plans needed

Qualifications & Experience:

• Active Nurse Practitioner License in the State of New York
• Current CPR certification (BLS)
• Board Certification through AANP or ANCC is required
• Master of Science in Nursing (MSN) from an accredited program
• At least six months of recent Nurse Practitioner experience (preferred)
