Provide essential home-based healthcare while making a meaningful impact through comprehensive risk assessments for Medicare Advantage plan members in Jefferson County. This per diem opportunity empowers Nurse Practitioners to deliver high-quality evaluations to patients in their homes, with flexible shifts and the ability to design your own work schedule.
Schedule:
- Flexible shifts available Monday through Sunday
- Set your own schedule
- Weekly or bi-weekly minimum hour commitment
Key Responsibilities:
- Conduct annual and post-discharge home health risk assessments for Medicare Advantage members
- Review and evaluate patient medical history, current health conditions, and medication lists
- Educate patients on their health conditions, medications, and preventive care
- Identify gaps in care and recommend appropriate solutions to enhance patient outcomes
- No treatment plan creation or medication prescription required; focus is on assessment and education
Qualifications:
- Active Nurse Practitioner license in Arkansas
- Board Certification through the AANP or ANCC
- Current BLS/CPR certification
- Master of Science in Nursing (MSN) from an accredited program
- At least 6 months of current NP experience preferred
In this role, youll visit patients residing throughout Pine Bluff, White Hall, Redfield, and other locations in Jefferson County. Each evaluation typically takes around 45 minutes, and you will receive fair compensation per completed assessment, as well as mileage reimbursement and administrative pay.
