Job Description
Make a meaningful impact on community health by performing in-home health risk assessments for Medicare Advantage members in Jefferson County. This per diem opportunity offers you control over your schedule and diverse workdays across Pine Bluff, White Hall, Redfield, and surrounding areas. Nurse Practitioners will perform thorough evaluations to enhance wellness, educate patients, and help address gaps in careno medication prescribing or treatment planning required.
Schedule:
- Flexible scheduling, Monday through Sunday
- Create your own shifts
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Complete annual health risk assessments in members homes, typically around 45 minutes each
- Review and evaluate comprehensive medical history and current conditions
- Assess current and past medication use with patients
- Educate members regarding health conditions, wellness, and medication regimens
- Identify care gaps and suggest actionable recommendations for follow-up
- Maintain thorough, professional documentation after each visit
Qualifications:
- Active Nurse Practitioner license in Arkansas
- Board Certification through AANP or ANCC
- Current BLS/CPR certification
- MSN degree from an accredited nursing program
- At least six months of recent experience as a Nurse Practitioner preferred
