Make a vital impact delivering healthcare in a home setting by performing comprehensive risk assessments for Medicare Advantage plan members throughout Columbia County, AR. Schedule/Hours:

• Flexible shifts available between Monday and Sunday
• Create your own schedule for optimal work-life balance
• Minimum weekly or bi-weekly hourly commitment required

Key Responsibilities:

• Conduct annual home health risk assessments for members enrolled in a Medicare Advantage health plan
• Evaluate patients prior and current medical history, medical conditions, and medication history during each 45-minute home visit
• Provide patient education about medical conditions and medication management
• Identify gaps in care and offer recommendations to resolve them
• Visits may vary, including annual assessments and post-discharge evaluations
• No requirement to create treatment plans or prescribe medications

Preferred Qualifications:

• Active Nurse Practitioner license in Arkansas
• Current CPR certification (BLS)
• Board certification through AANP or ANCC
• Minimum of 6 months of nurse practitioner experience preferred
• Graduate of an accredited MSN program

As part of this per diem role, you will receive competitive compensation, with $80$90 per evaluation, in addition to a mileage stipend and payment for administrative duties.
