Job Description
Deliver essential care right in your community by providing home health risk assessments for Medicare Advantage plan members across Columbia County, AR, including areas such as Magnolia, Waldo, and Taylor. In this per diem opportunity, youll leverage your Nurse Practitioner expertise to perform thorough assessments that help close gaps in care, all while enjoying the flexibility to design your schedule.
Schedule:
- Flexible shifts available seven days a week
- Build your own schedule to suit your availability
- Minimum weekly or bi-weekly hourly commitment
Responsibilities:
- Complete annual and post-discharge home health risk assessments for Medicare Advantage members in their homes
- Obtain and review prior/current medical history, conditions, and medication usage
- Educate members about their health status and prescribed medications
- Identify and address gaps in care by providing actionable recommendations
- Visits typically last about 45 minutes; there is no requirement to prescribe medications or develop treatment plans
Qualifications:
- Active Nurse Practitioner license in Arkansas (AR)
- Board Certification from AANP or ANCC
- Current BLS (CPR) certification
- Minimum 6 months of recent NP experience preferred
- Master of Science in Nursing (MSN) from an accredited program
