Advance your career as a Nurse Practitioner specializing in home health risk assessments across Columbia County, AR. This per diem opportunity offers a flexible schedule, allowing you to design your shifts between Monday and Sunday and select your weekly or bi-weekly commitment. Candidates may be scheduled for assessments in Magnolia, Waldo, Taylor, and surrounding locations within Columbia County.

Key Responsibilities:
- Perform comprehensive annual and post-discharge risk assessments for Medicare Advantage members in home settings
- Review and document patients prior and current medical history, medical conditions, and medication usage
- Educate members regarding their health conditions and prescribed medications
- Identify gaps in care and recommend interventions to address unmet healthcare needs
- Focus solely on assessment no ongoing treatment planning or prescribing required
Qualifications:
- Active Nurse Practitioner License issued in AR
- Board Certified through AANP or ANCC
- Valid CPR certification (BLS)
- MSN degree from an accredited nursing program
- At least 6 months of recent NP experience preferred
- Strong assessment, documentation, and patient education skills
