Job Description

This per diem opportunity offers a flexible schedule, allowing you to design your shifts between Monday and Sunday and select your weekly or bi-weekly commitment. Candidates may be scheduled for assessments in Magnolia, Waldo, Taylor, and surrounding locations within Columbia County.

Key Responsibilities:

  • Perform comprehensive annual and post-discharge risk assessments for Medicare Advantage members in home settings
  • Review and document patients prior and current medical history, medical conditions, and medication usage
  • Educate members regarding their health conditions and prescribed medications
  • Identify gaps in care and recommend interventions to address unmet healthcare needs
  • Focus solely on assessment no ongoing treatment planning or prescribing required

Qualifications:

  • Active Nurse Practitioner License issued in AR
  • Board Certified through AANP or ANCC
  • Valid CPR certification (BLS)
  • MSN degree from an accredited nursing program
  • At least 6 months of recent NP experience preferred
  • Strong assessment, documentation, and patient education skills
