Advance your clinical expertise by delivering compassionate home-based care as a Nurse Practitioner specializing in Home Health assessments. This per diem role centers on conducting comprehensive risk assessments for Medicare Advantage plan members located throughout Spotsylvania County. Responsibilities include traveling to various communitiesincluding Spotsylvania Courthouse and Lake Wildernessto provide essential evaluations and education for your patients, tailored to their needs.
Schedule:
- Flexible shifts available Monday through Sunday
- Ability to create your own schedule to fit your lifestyle
- Minimum weekly or bi-weekly hourly commitment required
Key Responsibilities:
- Conduct thorough annual home health risk assessments on Medicare Advantage members in their homes
- Review patients' prior and current medical histories, medical conditions, and medication use
- Deliver clear education to patients regarding their medical conditions and medications
- Identify care gaps and offer actionable recommendations to improve patient outcomes
- No prescribing or treatment plan development required
Qualifications:
- Current Nurse Practitioner License (VA)
- Board Certification through AANP or ANCC required
- Valid and current CPR (BLS) certification
- MSN from an accredited school of nursing
- Minimum of 6 months NP experience preferred
