Advance your clinical expertise in a flexible, rewarding environment as a Nurse Practitioner specializing in home health risk assessments. This per diem position allows you to design your own schedule while making a significant impact on Medicare Advantage plan members across Yancey County, NCincluding communities such as Cane River, Bald Creek, Celo, and Sioux.

Schedule:

• Flexible shifts available any day of the week, Monday through Sunday
• Choose your preferred hours and build your own schedule
• Meet a minimum commitment, either weekly or bi-weekly

Key Responsibilities:

• Conduct comprehensive annual home health risk assessments for Medicare Advantage members in their residences
• Review each patients prior and current medical history, medical conditions, and medication usage
• Offer clear education about health concerns and medications to empower patient understanding
• Detect care gaps and advise on potential steps for resolution
• Focus solely on assessmentsno need to create treatment plans or prescribe medications

Qualifications:

• Active Nurse Practitioner License in North Carolina
• Current CPR certification (BLS)
• National board certification via AANP or ANCC
• Minimum of 6 months experience as an NP preferred
• Earned MSN from an accredited nursing program
