Deliver vital care and make a difference as a skilled Nurse Practitioner specializing in home health risk assessments. This per diem position offers maximum flexibility, with the ability to create your own schedule and choose shifts throughout the weekincluding weekends. Serve Medicare Advantage plan members in McDowell County, where visits may occur in Marion, Old Fort, or other surrounding areas. Schedule:
- Flexible shifts available Monday through Sunday
- Design your schedule to fit your lifestyle
- Minimum hourly commitment per week or every two weeks
Key Responsibilities:
- Perform comprehensive home health risk assessments for Medicare Advantage members
- Review and document medical history, evaluate current conditions, and assess medication use
- Educate individuals and families on health conditions and medications
- Identify unmet health needs, gaps in care, and recommend appropriate follow-up
- No requirement to create treatment plans or prescribe medications
Required Qualifications:
- Active Nurse Practitioner License in North Carolina
- Current CPR/BLS certification
- Board Certification (AANP or ANCC)
- At least 6 months of recent NP experience preferred
- Master of Science in Nursing (MSN) from an accredited program
