A rewarding opportunity is available for a skilled Nurse Practitioner with experience in home health care. This per diem role focuses on conducting comprehensive risk assessments for Medicare Advantage members within Springfield and across Hampden County, Massachusetts. The position offers flexibility, autonomy, and the chance to make a direct impact on patient care in the home setting.
Schedule:
- Flexible shifts available any day, Monday through Sunday
- Build your own schedule
- Minimum commitment, either weekly or bi-weekly, based on your availability
Responsibilities:
- Perform thorough home health risk assessments, with each visit lasting approximately 45 minutes
- Assess and document prior/current medical history, existing medical conditions, and medication regimens
- Educate patients about their medical conditions and prescribed medications
- Identify potential gaps in care and offer recommendations to improve patient outcomes
- No requirement to develop treatment plans or prescribe medications
Desired Qualifications:
- Licensed Nurse Practitioner in the state of Massachusetts
- MSN degree from an accredited nursing program
- Board Certification through AANP or ANCC
- Current CPR certification (BLS)
- At least six months of current Nurse Practitioner experience, especially in home health, is preferred
